Hi Nancy,
The LSUS process is relatively simple. It only asks for approval from the Graduate Dean. As I am not an expert on individual programs, I ask the program director to send me a memo certifying that the courses are still relevant. I then send a memo to the registrar granting the exemption.
This process is relatively straightforward for certain areas where the passage of time is not that critical, but for others (e.g., computer science) I would hesitate to approve courses that are more than 10 years old (our limit is 8 yrs).

We approximately follow the early part of the process outlined by Eddie Lyons. The department head (or the coordinator of the graduate program) and associate dean for graduate studies of the student's college are asked to review and certify the courses. Their decision is conveyed to the Graduate School either in the form of a memo or an email. We also ask that a plan for completion of the remaining degree requirements and an anticipated exit date are both included in the communication to the Graduate School.
Memos received as hard copy (very rare these days) are scanned into PDFs, emails are saved as PDFs. In both cases, the records end up in the student's electronic folder shared between the Graduate School and the Registrar. Our Graduate Council is not involved in these decisions.

Right now, we require faculty from the appropriate department/subject area to confirm the student "demonstrated competency at a level of B or higher" AND the graduate coordinator to confirm this course is still relevant to the degree.
